    How about an answer to this man’s question? It’s 5 months old and the only question. Other than that, this is an excellent presentation of what I’ve been reading is a great alternative to other treatments and perfect it seems for me because of my BPH and large prostate and only one lesion, it’s location etc. Most likely excellent for many others of us and this Doctor has a lot of experience which is so important…

    Hi. I have a couple of questions. If I compare HIFU using fusion (marrying ultrasound to a prior MRI image) to FLA (with real time MRI-guidance), isn't the latter technique much more accurate? Why do you call HIFU the workhorse when compared to Tulsa Pro? Tulsa seems like it would be more precise (it uses MRI guidance I think), protects the urethra and rectum and the energy has to travel shorter distances (helps with larger prostates). I'm glad to learn you have done more Tulsa Pros that anyone in the country but trying to figure out why its your second-best tool. I'm very impressed with your recommendation for general anesthesia for keeping the patient still and comfortable and protecting the patient from being injured by delivery of energy to important structures. Thanks for being so passionate about your work.

    I've read conflicting opinions on whether the Tulsa Pro procedure may be covered by Medicare. I've read that it will be covered Jan 1st, 2025. Is this fact or just a possibility. I've also read that there are temporary CPT Medicare numbers that (55899 and 77022) that can be used now. I was advised by one source that only hospitals are currently able to use those temporary numbers. What is your opinion of this?

    I went through this procedure 9 years ago now. The catheter is normally suprapubic because the prostate is on the urinary canal so after the procedure they don't want to shove a tube up there.

    Eighteen years ago, I had a Gleason 5+5 then had radiation treatment with seeds at Stanford. My PSMA shows PCa in my sperm duct and 2 nearby lymph nodes. Is there a focal therapy to zap my sperm duct without damaging my bladder? How can I take out those 2 lymph nodes, one near my rectum?

    Hello, i have the option of doing HIFU or active surveillance . I want to go ahead & have HIFU, but i have a lot of anxiety about being catheterized. This should not hold me back, but is. Can u tell if the catheterization is painful or uncomfortable. Thx u

    After 11 years on AS and working with a Center of Excellence my cancer reached a point where it became prudent to consider treatment. Not surprisingly, my doctors recommended a prostatectomy or radiation despite having cancer graded as Favorable Intermediate. Not wanting to destroy the entire house because I had termites in only a few rooms I researched partial ablation and ended up focusing on Tulsa Pro and HIFU. I consulted with several doctors and ultimately selected Dr. Scionti who was experienced with both procedures. I did not want to be a beta test for other doctors who had far less experience. I had a HIFU hemi--ablation procedure just over 1.5 years ago. Thus far my one -year MRI follow-up and subsequent PSA tests have been very favorable. Anyone who is considering a partial ablation, and anyone who is considering one of the traditional approaches, should consider having a consult with Dr. Scionti to determine if they are a candidate.

    The question is, what is the criterion. Not all patient can benefit from focal therapy. I wish they he could be more specific.

      my research suggest the most important issues are if the cancer is localised and which part of the prostate it's in - not all Its can reach the tumor or if the prostate is too large that may also interfere. I agree I would like to know more about the therapy and less about the history

    One and half years ago Scionti Prostate Center ablated my prostate via Tulsa Pro. Dr Scionti is the DEAN of HYFU and Tulsa Pro treatments With 9 cancerous tumors Gleason Score 7 PSA 8 and rising I traveled to the beautiful city of Sarasota and glad I did.     Dr. Scionti, I have a localized tumor in the transition zone (Gleason 3+4 in only two cores out of 4 taken directly from the lesion in a MRI guided biopsy. Other 14 cores from the rest of the gland negative). I'm really considering focal treatment and TULSA sounds interesting. The fact that the lesion is in the transition zone makes it a better candidate for TULSA over HIFU? Isn't HIFU, overall, a more consolidated technology? I mean, being the patient and only interested in the outcome (and not the future possibilities of the TULSA Procedure), what I should consider to choose between the two treatments?

    I am a Medicare patient but I have elected to have Aetna Advantage as my insurance company...Does that mean they have to approve this even if Medicare provides coverage?
