What I'd like to know is how can the medical notes be that detailed when the doctor is busy with the examination and I don't see anything being recorded or written down. Also I've learned not to read medical notes, particuarly surgical ones. They're extremely scary and intimidating. At least the notes I get from my general doctor's practice are in mostly plain English.

Hello @tomservo75, Your observation is spot-on! It can indeed seem like a mystery how doctors manage to capture detailed notes during a busy examination, especially when it appears that nothing is being recorded at the moment. Typically, healthcare professionals are trained to efficiently document important details, often using shorthand or later dictating notes into a system. Some also use scribes or electronic health record (EHR) systems to assist with real-time documentation. I understand your concerns about reading medical notes, especially surgical ones, as they can be complex and sometimes unsettling. However, being informed about your health records can be empowering and help you in managing your healthcare more effectively. To help demystify medical notes and make them less intimidating, I recommend checking out our article on unlocking the secrets of medical notes through the SOAP note format, which can be found here: Unlock Medical Notes: Blueprint for Optimal Health. This guide breaks down and organizes the various types of medical notes and introduces the basics of the SOAP note format, making it easier to understand. Additionally, if you're interested in learning how to better communicate with healthcare professionals and even how to take and share your own notes with clinicians, our article on medical progress notes offers valuable insights: Medical Progress Notes: Learn the Hidden Insights. Empowering yourself with knowledge about medical documentation can transform your healthcare experience. If you have any more questions or need further assistance, feel free to ask!
