This Raven has a nest and is angry that I am on the roof and approaching it.
This is why I hate steam firetube boilers. Sometimes they can be explosive.
I used a Narvic air expulsion valve on my crane boiler, which greatly reduced the corrosion problem. At my last annual inspection the boiler was described as good as new, not 68 years old!

Should have kept 100+ppm of sulfite in the boiler water when it was off-line. This would have consumed the O2.
I am glad to hear that an alphabet soup of bureaucrats turned up to investigate the incident, in spite of great danger to themselves from the already exploded boiler. I hope they all got hazard pay.

I was wondering if it might be possible to get a copy of the video in its entirety? I think the video is informative about the dangers of boilers and even people who work with them everyday are not aware of. I would like to add this to on of my presentations to help spread awareness.
That's why bangers always burst lengthwise. Normally, the longitudinal seams are examined most carefully. I don't know since when boilers are made and monitored. Fundamental inspection points should be known today. Surely it's not my place but I think there went someting terribly wrong in the first place. They teach students?

There is no justification to not monitor the boilers, nor performing preventative maintenance. If you are an operator and lack knowledge and understanding about the equipment you operate, insist on training, and if it's not available, do self study. NO EXCUSES. In our present culture we've raised generations of people who think they are entitled to operations without self study, but it did not use to be this way. Train yourself and save your life!
I like how the video was saying: our kids, nurses, doctors, customers... and not once had any sympathy for the operating engineer. Down to one feed water pump in our plant.. wont fix the others. awesome.

So who was in charge of that boiler the instructor? What was he an instructor for? Someone who is teaching others how to operate and maintain equipment shouldn't have a preventable accident occur if he was doing everything the way he should be. Got to comfortable.
I listened to the commentary and heard the guy say "a De-air tank was not used, inline during the recyc stages of the low pressure steam generation phase. The de air tank phase shoots the low yeald steam or condensate against the inside of the tank to remove the air particulate. This caused, cavitation at low pressure, to form within feed pipe spools due to the steam turning from a vapour back to a liquid. The rust and pitting formed in side the firing/ heating tubes and along the weld. We know that when water mixes with air, we get oxidisation and rust forms. Metal fatigues when it is subject to high temps and contracts when it cools down hence a cooling shut off standard operation procedure..... the pitting along this weld was where the cavitational steam bubbles exploded weakening the internal tank structure. We hear him say this "it was weakest at the the point of failure due to the long periods of rest and quick periods of operations. and the failure to use a de air filter/tank...... and if they used salt or fresh water the chance of rusting, cavitation, or pitting would have been higher...... just saying
What an amazing project.
Amazing an element we have used since almost the dawn of man and yet we still have accidents with it. Just shows no matter how old the element or technology we still must respect it.

i still work with clever brooks four pass boilers just like this one. if people are going to run boilers of this age there needs to daily as well as monthly inspections.they are good boilers and last alot lot longer if you use them in a low pressure application .

That's why a chemical oxygen scavenger is used. and in this case there was no chemical being added to the boiler water.
O2 scavenger should have been used.
That was done in 1993 - 10 years earlier of the incident.. if im correct?

So on thing that should be done for these boilers that are constantly cycled on and off in school without the use of a "De-A", should be more frequently inspected thoroughly. At my school, we developed and tube leak in our Cleaver Brooks Water tube boiler operating at about 120lbs. That was enough of a close call - to see water leaking from the boiler furnace. A close enough call at school for me. Another example of a constantly cycled boiler. Ver y serious measures should be made.
Strange that an annual hydrostatic test to 150% operating pressure would not have caused a rupture before firing, but never know with cracks.

If the boiler had been flooded with air deprived water, yes this could have been avoided. It is also noted in the video that an air dryer if it had been used would have also reduced the risk of oxidation of the vessel.
can there be liquid flyash inside the boiler that through the sight glass look like a liquid? just curious. please reply. and is it something to worry about? one side is empty one side has the liquid u see in the sight glass.
