@Mirko von Hein: Thank you so much for this video. It was a great demonstration of the Markov model. I am a modeling naive HEOR professional and had a few questions. Do we still use Excel for the Markov model, or is there other software available? What happens if the drug is a rare disease drug and you do not have a lot of patient data available? Do you only use iterations using VBA, or is there a different way to get data? I know one limitation of the Markov model is that it's more accurate for chronic diseases. Could you maybe talk about it more? Also, can you demonstrate a decision tree model in Excel? Thanks a ton for sharing such useful content , I wish I could get to do modeling.
Thanks for your comment & all the important questions you raise! Let me try to answer these one by one. - Modelling Software: Yes, Excel is still the the most widely used tool by far to create cost-effectiveness models. Other solutions exist (R, TreeAge, Python), but they're not as popular. - Rare Disease Modelling: We have to make due with what we got! No model is ever perfect, and if the data is "BAD" in the first place, this is never a good place to start your modelling from. However, given the rare nature of some diseases, we simply won't get large datasets of thousands of patients, so we have to use what we got. - VBA: Visual Basic for Applications is Microsoft's programming language that's built into the MS Office Suite of products. For CEMs, we use it to do deterministic and probabilistic sensitivity analysis, and maybe add some user-friendliness features. - Decision Tree Model: Yes, that's on my list! :)

This was a very good explanation, each time you come across a Daily , chances are it's confused with Qaly.Although I didn't catch the disability weight part in the formula .
Agreed, the two concepts are often confused! Disability weights (DW) represent the severity of a health condition. The more severe the condition, the higher the DW. For example, a stroke with no recovery would have a very high DW, whereas a minor hearing loss would have a much lower DW (for reference: in my video I used 0.4 and 0.8 as the DW). The DW is captured in the years lived with disability (YLD) part of the DALY equation, which also considers years of life lost (YLL). DALYs are calculated as YLD + YLL, whereby YLD equals the time an individual experiences the health condition multiplied by the DW. Another good way of thinking about it is like this: QALY: “0” is equivalent to death; “1” represents perfect health. DALY, “0” represents perfect health; “1” equates to death. Hope this helps!

Great video! Flexible working for consulting, in my opinion is more of a norm nowadays (in the US imo). Wanted to add that a balance in pharma in terms of in-person/remote could be a pro for a mixed intro-extrovert 😊
I agree flexible working has become quite standard in recent years, which-in my opinion-was long overdue! For me, a balance between in-office days and WFH works best. I find that I get more stuff done when I’m at home, but seeing people in the office creates personal connections which ultimately contributes to a happier work environment. I’m fully aware, though, that there is no “one-fits-all” type of approach, since we all have different requirements to work around.

Thanks for your concise video explaining about CEM, it's very useful especially for me who is starting to pursue my health economist career. I just want to know about PSA, do we need to use R to create this model or excel is enough? How about Dynamic transmission model? I am in the process of learning R at this moment but I still cannot use that to develop my model. Anyway, may I know where to learn another model Mirko? I learn about markov and DT during my master degree at LSE but I haven't been tought another areas eventhough I believe this will be beneficial to nail a job related to modelling.
You can use either Excel or R to create any of these models. Both can generate the results you need, including PSA! Excel is more widely used than R, but learning R is a great skill to acquire as a health economist, so I’d highly encourage you to keep up with that! If you’re keen to dive more deeply into modelling, you could get a copy of this book, which has some practical examples in it: www.amazon.co.uk/Effectiveness-Modelling-Health-Technology-Assessment/dp/3319157434 Alternatively, @TMSnowsill also has some great content on his YT channel that might be relevant for you. This is good, but I guess the full length videos are at a slower pace and help understand the concepts better. Could be just my old school self
I enjoy creating longer-form videos more myself, but I believe there is also value in having condensed formats like the YT short to complement the longer videos. Honestly, I’ve partly started switching to YT shorts to maintain weekly uploads, especially over the holiday season. These YT shorts are quicker to produce and give me more time to focus on videos that will take longer to create. People always have different ways of learning / absorbing new information, so I think having both shorts & long-form videos doesn’t hurt 😊

And during the whole process the government is providing tax payer dollars to companies to mitigate those massive losses. Not a single drug has hit the market in the last 30 years where we the tax payers haven’t footed a large part of the R&D expenses.

@Mirko von Hein: This is the most required skill in any area, especially in consulting. With AI coming in, highlighting the importance of creating presentations is relevant. You can get the templates, and the design and images using AI, but the flow of the presentation is to be decided by humans. Thanks a lot, great content. Please keep sharing such valuable content.
I'm sure AI soon will be able to give suggestions for how to improve storytelling in presentations as well... I haven't actually used AI much for PPT slides. I'm curious, what AI are you using for your slide decks?

How is the quality of life rating determined? A mix of patient reports and analysis of side effects?
QoL can be measured using generic or disease-specific instruments. Common examples of a generic QoL instruments include the EQ-5D and the SF-36. Both are questionnaires that are completed by the patient, which cover various domains. Disease-specific QoL instruments on the other hand are tailored to specific conditions, like the EORTC for cancer for example. On top of these two methods, you could also use direct utility elicitation methods like standard gamble or time trade-off to capture QoL, but using standard surveys, ideally captured within the primary trail of a new drug, is usually easier and therefore preferred.
