I think this test is nothing more than a joke I have elevated PSA My urologist wanted me to automatically do a random biopsy I said hell no I want MRI He said okay well let's do this urine test and so I did and it showed that I had a increase likelihood of prostate cancer so I went ahead and MRI and MRI didn't show absolutely nothing this was two years ago I already had another MRI and still nothing not only that but when I did this urine test I woke up in the morning drink tons of water send the sample off which look like nothing more than water so it was extremely diluted just to see if they will still test it and they did test it and they said a bunch of BS

In my opinion this urine test the free PSA test they're all indirect tests to try to provide some answers if or if not you have prostate cancer just go and get in MRI and that'll show you

What unethical cowards who don't have the integrity to say CHEMICAL CASTRATION, but hide behind the misleading euphemism ADT. Bet you don't warn your patients that up to twenty percent of older men NEVER RECOVER ABOVE CASTRATE LEVEL and after 18 months or more of ADT CASTRATION, 75 percent of all men WILL NOT RECOVER THEIR TESTOSTERONE LEVELS EVEN AFTER A MEDIAN TIME OF FIVE YEARS.

I had a stroke on 7/8/23. I'm going to a stroke gym twice a week for recovery, but no matter how hard I train, it's a bitch to regain strength on my good side. I'm on a carnivore diet, and my BP is under 110/70 every day. My T level is 385, I would like to bump it up to 800. Would that increase my risk of another stroke?

98% ignores the fact that the patient still has to undergo a complete separate treatment. This is after they already agonized over the decision about whether to pursue surgery or radiation in the first place. I can't imagine it would feel like much of a wind to find out I still needed radiation after undergoing a radical prostatectomy and suffering all the side effects associated with that.

Fired two different Urologist. The first scheduled me for two months out, his staff said MRI is not available for a month, had a piss test that came up Cancer, Cancer, Cancer. Had a conflict in schedule with MRI. Called to reschedule, a MRI was available that day. The staff lied. The second urologist took a little long with biopsy, then CAT scan, Gleason 4+5. Then he wanted to schedule a telephone call two months out. Good-by to the second urologist. Reviewed all needed information on you-tube with Dr. Sultz of the Prostate Cancer Institute. Getting cut seems insane when the cure rates are about the same for radiation and surgery. Went to a Center of Excellence in Philadelphia, Fox Chase Cancer Institute, THEY ONLY DO CANCER. Got an appointment in five days and had four more appointments in the next week. Got nuked with 26 appointments and am on first and second generation hormone therapy now. I did not miss even one day driving the students to school. Can you say that about being cut? Advice? Go to a Center of Excellence where ever you are. Review Prostrate Cancer Institute information on you-tube. Don't put-up with Urologist wait, wait, and wait. You will loose you sex life almost always. That is just how it is. Well, on hormone therapy you will not even care for sex. It sucks but accept it. You are suffering from concerns? If you have done your homework and have the best care possible, don't worry. You have done your very best and what will be will be. Worry about it make it worse. I wish you the very best of everything and "Good luck with everything you are going through." Sorry to hear you are having a tough time.

VA endocrinologists should be embarrassed for themselves. I had a very post menopausal endo attending try to tell me what I SHOULD feel like as an early middle age male, and then went on to blow smoke up my ass telling me I should feel perfectly fine at a T level of ~200 (and while significantly symptomatic). TRT is LIFE CHANGING therapy, and I probably would have suck started a shotgun after getting out if it weren't for the TRT I paid out of pocket for. VA endo and endo in general = clownshoes.

Love the discussion! Two great respected minds excellent questions! I am curious to find out as your discussions around collecting data from focal procedures, why don’t the speciality clinics like Sperling Medical whom have performed thousands of focal ablations, are they not required to provide follow up data to the academic institutions so that the knowledge base for cure or recurrence can be updated and allow for more widespread acceptance of focal offerings. I am in the process of deciding on a focal laser or cryo procedure. I just turned 65 so I learned that I would be covered under Medicare, vs going to Florida for an out of pocket expense.
