Hello po. I would like to get some advice in filling out section 1. Applicant Information of the NYSED form 2F My problem 1 . is the start of my name which has MARIA in my passort and birth cert. but in my diploma, TOR & PRC ID the MARIA is shortened to MA only. Should I put the full MARIA based on my passport&BC or the shortened MA based on my diploma/TOR/PRC ID? 2. For NYSED form 2F question #8: asking for the date of attendance (from mo. /day/ yr to mo./day/yr ) will the informations be provided in my school? 3. For form 3F for question #4 Print name I will put the full MARIA shown in my passport and birth cert. While for question #8 If you took a licensing examination using a different name. Should i put the shortened Ma because bec it is whats in my prc ID and diploma? 4. Is there a need to make an affidavit of the one and the same person because of my MARIA and MA name difference in my Passport/BC vs Diploma/TOR/PRC ID? In uploading those files, di na po need yung elementary diploma? Paano po if K-12, need po sa Junior & Senior High School Diplomas?

Hello po! What if 2 schools ang inattendan po? 1st-3rd ko from a different university then i transferred on my 4th yr at a different university naman. Hindi po ba magkaka discrepancy? Kasi yung nilagay ko lang sa dates of attendance is yung last year ko (4th yr) hindi kasali yung 1st-3rd yr ko.

Hello po, sana po matulungan nyo po ako mejo nalilito na po ako, kasi po nagprocess na po thru agency tapos na po ako sa step 1 for step 2 na ako for ATT na po kaso I learned po na ang mahal po pla sa agency around 400 dollars, while kung mag diy po 200 dollar lng po, malaking tulong na po sa akin kapag nakaless po ako sa gastos kasi wlang wla tlga ako ngayon di abot ipon ko nang 400 dollars, kaso po di ko po alam san at paano magstart sa diy po for att..
