Infraclavicular brachial plexus exposure
Exposure of the infraclavicular brachial plexus is necessary to access the cords of the brachial plexus and its branches. It provides access to terminal branches of the posterior cord, the axillary and radial nerves, prior to passing through the quadrangular space and triangular interval. Within this region are potential nerve donors such as thoracodorsal and medial pectoral nerves which can be used to reinnervate the axillary nerve and free functioning muscle transfers to restore elbow flexion. This video provides a demonstration of obtaining a broad exposure of the infraclavicular plexus by taking down the pectoralis major muscle and its subsequent repair.

    I had this surgery done about 8 years ago. It went very poorly. For some reason that I still don't understand, there was no follow-up ultrasound completed closer to my surgery date, so they were relying on the one that was completed for my initial diagnosis 7+ months prior. While the doctors were in there doing their thing, and eventually cut and pulled my Gallbladder out, the Doc accidently tore a dime size hole in my stomach, and then had to quickly convert to emergency open surgery to fix me up, and quickly clean up the leaking stomach acid. They explained that my Gallbladder had become so large and inflamed that it somehow fused to my stomach. I've tried to research how common this is, and haven't found much, so I'm guessing it's pretty rare? So my surgery that was supposed to have had me home by suppertime, ended up keeping me in the Hospital for a week. I'm so super grateful for the staff there. I was in a lot of pain, and therefore on a lot of pain meds, and they tolerated my obnoxiousness very well, lol. I don't know why, but as soon as I was woken up post-surgery, my stomach muscles kept trying to clench up together (kinda like when you do a sit up), it was excruciating! My abdominal muscles had been bisected on an angle, the cut was almost a foot long. I'm not sure if this is the normal cut they would do for a planned open Cholecystectomy, or if the doctors just decided to connect the existing holes they made for the initial laparoscopic surgery. I wonder because when I look online for scars from planned open Cholecystectomy surgeries, they don't look as big as mine. Any Medical Professionals out there have any idea? Happily, after 6 weeks off work, I was feeling much better. Luckily I have had no major issues since losing my Gallbladder, except that I have a big area on my belly under the scar where the nerves were damaged, so the skin is super numb, but also still hurts internally if I put pressure on it, so it's something I have to be conscious of. I also think that some of the scar tissue from the would attached to a chunk of my bowel as I feel it in that spot as "stuff" moves through. Weird, but not painful thankfully! I was SO happy to be able to eat food with fat in it again! My first post-op poutine was heavenly, lol. The whole time after my first Gallbladder attack, all the way to my surgery 7+ months later, I couldn't eat anything with fat as it would cause pain (even just a piece of lunchmeat). I DO NOT want to put anyone off having this surgery done if they need it. It's definitely worth it to be able to eat normally again. What happened in case is apparently pretty rare. Ironic story: When I had my initial consult with my surgeon, he said that only about 1 in 2000 Laparoscopic Cholecystectomy surgeries end up having to move to open surgery. My hubby literally jokingly asked "So, are you due?", which gave us all a nice chuckle at the time...little did we know. Also, something that I though was interesting was when my surgeon asked if I had ever had a child naturally. I hadn't, and asked him why, and he said that he had heard that a full-on Gallbladder attack is more painful than childbirth. SO if anyone out there reads this and can answer that question, please do! I'm curious too! My first big Gallbladder attack was by far the worst physical pain I had ever endured up to that time, until, of course, when I woke up after surgery with the muscles clenching the way they were. Oh, side note: my husband said he could tell when I was about to wake up from my dead sleep because my heart monitor would suddenly start going nuts, and then I would wake up crying out in pain and pushing my med pump button. I found that kinda interesting. I wish he had taken some video on those first few days while I was in the hospital, I would've liked to see some of the things that happened, and some of my dopey behaviour, afterwards. Lol.     I just had this done in January. The only 2 hiccups were that the removal cut had to be made a bit bigger, and my gallbladder was fused to the bottom of my liver. All is well, and I feel so much better. Edit: one little stone? Mine was a freakin' hackey sack! 😅

    I had my gallbladder removed 2 weeks, 3 days ago. Excellent Dr and his team. I was sent home just 1 and a half hours after surgery. No pain, to speak of after. Easy peesy.

    Very cool to watch this.I had my gallbladder removed and when the doctor went to go in through the umbilical, he found a hole. He told me after that I had a hernia which I didn’t even know. So, he said, they went through the hole and closed it on the way out lol 😂

    Wow!! I happened to get this in my feed to watch. I had my Gallbladder out by this method 20+ years ago. I fell ill at work (Friday lunchtime) and was taken to A and E, they did test after test and couldn't find out what was wrong with me, so eventually discharged me with Morphine and Pethidine for over the weekend!!??, and was told to see my GP on the Monday. I put in a poor weekend and got a Drs appt. She listened to my saga, then got me up on her bed to examine me and when she pressed on my gallbladder I just about hit the roof!! She diagnosed Gallstones and probably a bigger on stuck in bile duct. She sent me back to hospital for a scan and yes the gallbladder had about 20 small stones and a larger one stuck!! I eventually got the gallbladder removed and the day after surgery I heard the surgeon doing his rounds. When he got to our ward of 4 he looked in and then looked puzzled!?, I heard him asking who I was?? Nurse replied with my name and pointed at my file, that's not Mary ....... that I operated on yesterday!!!, next the door open and him and the Ward Sister came in, she says to me, 'whats your full name Mary and your date of birth please?', he was more than gobsmacked!! Best ever patient day after surgery!!! He checked my belly and had a look at the belly buttons dressing, all ok. 'Do you want to go home today?', 'yes please' I replied!.     When I had my gallbladder removed 46 years ago, it was abdominal surgery by my GP with big hands! I feel lucky to be alive after seeing this much more sophisticated technique, I had loads of adhesions to my liver and duodenum and rough stones in the gallbladder and bile duct. I now have adhesions at the incision and a misshapen abdomen from his big bite abdominal closure. Very interesting to see laparoscopic procedures!
