Hello 👋, your concern is absolutely true… technically if the Caries didn’t pass the level of the alveolar bone crest, one could try to be conservative and attempt to raise the interproximal margins to restore, but in most of these cases, the lesions are way below the gingival tissue level and then if it is not restorable like that, it falls in the inclusion criteria of extraction as you said! 👍🏻

@@understandingdentistry I think it may be a good option, although it is one of those treatments that requires a real time decision (depth of decay, prolonged haemostasis etc). With CSCs you can get great results, but obviously RCT's are very safe and predictable option. So it's difficult to move away from a treatment that is so routine and practiced

In this case, root canal treatment was required as clinical examination revealed negative result for the vitality test, as mentioned in the video…

Hello, so as you noticed the Caries are now approaching the root area. Although one can try to be conservative and restore, it will be challenging to keep this tooth after removing this root Caries. However this case was used to discuss whether or not this proximity to the pulp chamber would lead to root canal treatment, and it usually does, in cases like that…. Take care

The radiographic diagnosis is mixoma due to the reasons that I explained. Then, the lesion was removed and sent for histological analysis, and it was confirmed the diagnosis of mixoma.
