Candidates for TOLAC/VBAC - Ideal candidate - 1. Previous 1 LSCS 2. Previous 1 LSCS with a vaginal delivery 3. Current singleton pregnancy TOLAC is reasonable - previous 2 LSCS TOLAC is Acceptable acc. to ACOG, but individualise Management 1. Previous 2 LSCS. Risk of uterine rupture in women with more than one cesarean delivery is between 0.9% and 3.7% 2. Suspected macrosomia alone should not preclude offering TOLAC. 3. Gestational age greater than 40 weeks alone should not preclude TOLAC. 4. Previous low vertical incision 5. may choose to proceed with TOLAC in the presence of a documented prior low- vertical uterine incision. 6. women with one previous cesarean delivery with an unknown uterine scar type may be candidates for TOLAC 7. women with one previous cesarean delivery with an unknown uterine scar type may be candidates for TOLAC, unless there is a high clinical suspicion of a previous clas- sical uterine incision such as cesarean delivery performed at an extremely preterm gestational age. 8. Women with one previous cesarean delivery with a low-transverse incision, who are otherwise appropriate candidates for twin vaginal delivery, are considered candidates for TOLAC. 9. Women who have a BMI of 30 or greater may be candidates for TOLAC RCOG - Individualised assessment for suitability for VBAC Factors that potentially increase the risk of uterine rupture include 1. short inter-delivery interval (less than 12 months since last delivery), - also increases risk for preterm delivery 2. post-date pregnancy, 3. maternal age of 40 years or more, 4. obesity, 5. lower prelabour Bishop score, 6. macrosomia and 7. decreased ultrasonographic lower segment myometrial thickness. Further data are needed before the safety of such an approach can be confirmed. Presence of these risk factors does not contraindicate VBAC Planned VBAC is contraindicated in * • previous uterine rupture * • classical caesarean scar * • Transfundal surgery * • Prior open maternal fetal surgery * • women who have other absolute contraindications to vaginal birth e.g. major placenta praevia * • insufficient evidence to support the safety of VBAC in women with previous inverted T or J incisions, or significant inadvertent uterine extension at the time of primary caesarean - caution Other conditions 1. 1. Short inter pregnancy interval * a. ACOG - (<19 months) associated with a reduced chance of achieving VBAC * b. RCOG - (less than 12 months since last delivery), - increased risk of uterine rupture, and also increases risk for preterm delivery * 2. Previous preterm CS - not mentioned * 3. Current preterm - RCOG - planned preterm VBAC has similar success rates to planned term VBAC but with a lower risk of uterine rupture. * 4. Type of uterine closure - not mentioned in either guideline, articles quote increased risk of rupture with single layer closure but non mention that this is a contraindication to VBAC * 5. Scar thicknes RCOG - According to a study - myometrial thickness (the minimum thickness overlying the amniotic cavity at the level of the uterine scar) * • cut-off of 2.1-4.0 mm provides a strong negative predictive value for the occurrence of a uterine defect during VBAC, * • cut-off between 0.6 and 2.0 mm provided a strong positive predictive value for the occurrence of a uterine defect
