Hi Allison, new grad RT here. Question: if let’s say a baby’s sat was dropping to as low as the 30s, and I wanted to manually give PPV (using a jr bag), can you explain how you’d go about it? Would you turn the jet off and just provide normal breaths or would you just hold PEEP? Can you explain the step by step process. Thank you

A rate on bipap does nothing. If patient isn’t breathing, the bipap won’t breathe for them. They need intubated

So what is the difference between the Ballard and the Surfactant Catheter? Can you use a Ballard to administer Surfactant as well?

If you have a trach patient, PLEASE DON'T USE THE ARCHAIC COOL LVN!!! Do them a kind favor, and use the Heated LVN!!!! Trach patients deal with this at home. These cheapo DMEs send them home with disguisting bacteria filled Cool Large Volume Nebulizers, with these archaic loud compressors, and they expect them to use these, instead of a HEATED set up. So, it becomes a real nightmare to get warm humidity. Why do you think I just ditched anything but the ventilator? I don't use the compressors. I don't tolerate cool mist, yet i couldn't get warm humidity except on my vent. So if I'm wanting to do spontaneous breathing for a few minutes, I go to Pressure Support, PEEP 5, PS of 7 and 5 L/min of O2. BECAUSE MY DME SUCKS!!! Loud compressors. No heate humidification. and there's that cold bacteria filled water sloshing around in the tubing, and that disguisting water trap!!! Come on people!!!! What happened to quality healthcare? Isn't the goal suppose to be give a home patient the same quality of care as in the hospital? It wasn't our choice to end up on long term tracheostomy support. We're on, or off vents, and have trachs. We've been through enough as it it is, and we've often been so sick that we've CODED!!! Had rapid response team call on us. We've been on the vent for months in ICU. And then we're in the hospital, on a nice ventilator, and get sent home on the standard portable vent, and if we want to get off, what do we get? Oh, we get cool mist! COOL MIST!!! We get a huge bulky loud compressor with a gauge (THESE THINGS ARE NOT DIGITAL), They're archaic. And these compressors are heavy, loud, and AND the cool humidification has barely enough flow to give us propper humidification, so we STILL get plugs. We have this tiny little water trap bottle you have to empty, because that bacteria filled cold water goes in there. You want infection? There ya go. Lots of infections for you!! Lots of gross bacteria going EVERYWHERE!!! I stopped that a very long time ago!!! My compressors are somewhere in this house, I don't remember where. I DON'T CARE!!!! There somewhere that I'm not going to see them! If I feel like want some spontaneous breathing, "CPAP/PS," with Low pressure support!!
