It doesn't suit him because Karamatsu is way more responsible, courteous, and big brotherly than Osomatsu will ever be.
And you are wrong about this. Osomatsu actually acts as the eldest when the situation needs him to. You can see many moments like that. Don't forget that it is Karamatsu that usually gets ignored by the others while most of the time, the brothers follow what Osomatsu does. The rest of them see Osomatsu as a big brother while Choromatsu and Ichimatsu don't even call Karamatsu "nii-san". Even Karamatsu's voice actor, Nakamura Yuichi, said that Karamatsu actually doesn't do much for his brothers. If you wants to talk about being responsible then it would be Choromatsu, although he tried and failed.

In some interviews published in magazines, the reason Karamatsu doesn't call Osomatsu "nii-san" is that he doesn't have much respect for Osomatsu, and when Osomatsu fails to act the eldest, Karamatsu will step in.
