The horizontal line represents the flat faced mushroom follower. The diameter of that follower is given. So that line represents the diameter of the flat faced mushroom follower.

SIR, WHY WE TAKE SMALLER GEAR AS DRIVER AND BIGGER GEAR AS DRIVEN ?
Dear, Generally the gear drives are used as speed reducers. This is possible only when the smaller gear is a driver. So, in general practice if it is not specified, then a smaller gear (pinion) is considered as a driver.
