Feline Hyperaesthesia Syndrome (FHS) - video of affected cat
A 7-year-old male domestic shorthair with suspected feline hyperaesthesia syndrome. Signs described by the owner included: twitching of the skin, ears and tail with vocalisation; high speed running; leaping into air at “invisible things” and hiding. The owner thought that wind and anxiety was a trigger (especially anxiety to other neighbourhood cats). The cat responded to pregabalin medication and environmental modification.     My dog was diagnosed with autoimmune brain inflammation six months ago. In addition, she had minimal brain herniation. In November, one night she was pacing and restless. I took her to the vet thinking she had a relapse. They gave her serums. My maltese developed very severe head tilt and excessive drooling after taking Mannitol and corticosteroids serum. When I take her to the vet she was able to walk and conscious. Within a few hours after taking those serums she had severe neurological symptoms and not able to walk either. We called the vet and she gave her Mannitol serum again. She died in 3 days in pain. She was only 3.5 years old. Did those serums kill my pup?

      Sorry for your loss - must have been extremely distressing. Sounds like meningoencephalomyelitis of unknown origin (MUO) which is a very serious often fatal disease that is treated with aggressive immunosuppression. (see here from a colleague's website vetoracle.com/resources/client-factsheets/meningoencephaltis-of-unknown-origin-factsheet/#:~:text=Meningoencephaltis%20of%20unknown%20origin%20(MUO,certain%20parts%20of%20your%20body.) I am sorry that I don't know what serums is (unless you mean straightforward mannitol which is used in an emergency setting to reduce brain swelling). Sadly I think it more likely that your poor dog died from the disease.

      looks interesting, new developed drugs can be prohibitively expensive for pet owners when first released and I have yet to see any pharmacokinetic data for dogs/cats so we shall see.

      It is not clear for what disease or process you are referring as this video covers a few (i.e. help what?), however collagen supplements may be benefit joint diseases (i.e. osteoarthritis) but this video is on changes (not disease) and diseases that you might see on spinal radiographs- so no I would not recommend this supplement for spinal disease because this supplement is for cartilage health not intervertebral disc health.

(that was the point of the video - but to be fair that message is directed at vets). however chondroitin sulphate and glucosamine supplements may help with osteoarthritis associated with hip dysplasia - I suggest that you look at the resources at canine arthritis management caninearthritis.co.uk/ and that should help you optimise the care for your dog

    An observation about the weight-gain aspect - completely anecdotal - my kitty is a big boy and he was initially given this drug after tooth removal and I noticed a huge improvement in how he coped with back pain. My vet was concerned about weight gain long-term, but when he is on the drug he is more active, and when he is in pain he comfort-eats. There was actually a notable weight loss on the drug. These days I give it to him only when he is having a really bad day to offer some relief.

    So helpful and interesting. My Cavalier, Misha, was on phenobarbital for 3 years but my vet didn’t do any monitoring at all! Misha was on the heavy side to begin with but within two years of being on this med he’d put on 3 kilos! So I asked my vet to put him on another epilepsy med (the expensive one) but it seemed to make him agitated and he was still obsessed with food. So now I’ve taken him off all epilepsy meds. He only has a fit about once a month and the Gabapentin he takes for SM really helps with keeping them short - usually around the one minute mark. I time them and keep a seizure diary and always have rectal diazepam at the ready in case the seizure goes on for too long. But as Misha is also on steroids and Gabapentin, both of which also cause weight gain, I felt that the increase of appetite caused by the epilepsy med was more of a threat to Misha’s life than allowing him to have the actual seizures.

    Thank you for this! My boy Bilbo (a Maine coon) was referred to the incredible team at Dick Whites and tested positive for the LGI1 antibodies. He was showing signs of hyper salivation, involuntary urination, unaware of his surroundings and vocalisations. He responded incredibly well to a course of prednisolone, phenobarbital and leviteracitum and went 3 months without seizures. Unfortunately the seizures returned when we tried to lower the dose so he’s back up to his normal dose of Leviteracitum and phenobarbital and we are trying a course of prednisolone again whilst we wait for his bloods. He has the odd seizure every now and again but they are nowhere near are severe or frequent. (He was having 5 a day before his referral). It’s good to know we aren’t alone and that Muddis is doing okay ❤

    Thank you for sharing this information. Harry is currently being treated for suspected limbic encephalitis. He had h8s first seizure on Friday and managed to get into Liverpool small animal teaching hospital on Saturday. His MRI on Sunday shows possible limbic. We are waiting results of the blood test. We are struggling to get his seizures under control at the moment, but after reading about Muddis I feel a little more optimistic. Thanks again.

    Hi Professor! Thanks for such an informative video. I was searching everywhere for some information regarding Pregabalin dosage for cats with FHS. We transitioned from Gabapentin at 100mg twice daily several months ago as my cat started refusing to take the meds in her food. We commenced Pregabalin at 25mg twice daily and in fact, her symptoms seemed improved. That was until the last week or so when she started tail biting/attacking again. I called my vet today who said to increase her dose to a total of 75mg daily (either 25mg x 3 or 3 capsules split into two doses). I’m concerned that this is too high a dose? She weighs around 6.3kg (gained weight since starting Pregabalin 🫣) I did give her a second 25mg dose today around 8hrs after the first with a plan to give her a third one before bed, however the second one really made her quite sedated/sleepy? Is this normal? She was never this quiet with the 2 x 25mg daily given roughly 12hrs apart. Thanks so much for your invaluable advice 😻

    Hi we have a 9 year old Chihuahua that was diagnosed by MRI having Chiari malformation. The doctor has advised against surgery and our dog is now on medication since 5 weeks. The medication is 1x 5 mg dermipred, 1x 5mg Losec-mups, and 2x 25mg Lyrica a day. In the beginning of the treatment we saw a big improvement in his condition when dermipred 5 mg was given. His condition was the he lost like 70% of his strength from one minute to the other and could barely stand on his feet, Falling regularly on his face while walking. Since dermipred i would say his strenght is now at 50% but he is not getting stronger. We also tried 3x gabapentine instead of the 2x 25mg Lyrica when we started the treatment but it seems that while his condition improves, it does not seem to be enough to get him trough a full day without pain. I am reaching out to everyone who has expertise on this terrible disease to get as much information on how to improve his life. All information is very welcome thank you.

    Thank you, Claire -- very nice summary. I will add that over the last 14 years we have not had a repeat surgery. I do exactly what you illustrated here (FMD, duroplasty, and SIS). I suture the SIS over the duroplasty defect and then place an autologous fat graft over the site. Like you, we don't have all the follow-up we would like, but in a coupe of cases, the repeat MRI at 3 years has shown the fat graft in place. Significant reduction in the syrinx, as you have said, is uncommon, but improvement is not uncommon -- at least on all of our follow-ups, we have always seen improvement -- usually with medications, sometimes not. I will say that we have seen a reduction in the syrinx in patients we perform this at less than 2 years of age. Many thanks! Andy

    Thank you so much for this information. My 6 year old Cavalier 11 kilos developed SM about a year ago, we don’t have MRI available in Tasmania and it would be very expensive and not worth doing unless you were having the surgery done. Willow has been on Gaberpentin 100 mg and ended up having 5 or so capsules a day for about 10 months. Another vet recently changed her to Pregabalin 25 mg capsules 2 twelve hours apart, as she had some breakthroughs we give 2 at night and 3 spread out through the day but Im wondering if they are better given two together maybe 3 time in 24 hours. She dosen’t yelp in pain but does head rubbing body twisting and some scratching. Still likes her walks etc and quite bright but yes has put on a lot of weight, doesn’t have heart trouble at the moment. Unfortunately my vet has not had a lot odd cases of SM so prefers if I do my own research. Thank you Liz.

      I am afraid that I cannot give advice on medication to individual patients that are not under my care although I get that you have been told to do own research. However your vet is able to contact me for (brief) advice though the usual channels (for vets). I will say that the dose of pregabalin is typically half that of the existing gabapentin dose although MAY be able to give less frequently ie 150mg gabapentin three times daily = 75mg pregabalin twice or three times daily.

    I want to thank you so much for this beautiful presentation... I am so grateful that one of you genius' has found a way to spell this out in laymen's terms..... I am currently fighting off any signs of suffering in my 13 year old Cav with gabapentine prednisone, prilosec, and CBD. As well as treating her heart murmur with cardiac meds.... I refuse to allow her or any of my sweet pups suffer but the comfort measures can only take us so far before we need to make a hard decision...

    Such a helpful and interesting video! Do you think that dogs with idiopathic epilepsy who used to have seizures no more frequently than once a month and who have now put on excessive weight due to Phenobarbital should be switched to Imepitoin? A lot of vets who put epileptic dogs on Phenobarbital don't ever bother monitoring their blood or checking their livers, so maybe it would be a safer option too in that case?

      Phenobarbital is more effective than imepitoin and there is a huge risk in switching a stable dog Having (sadly) seem a lot of cases where it went badly wrong I would not undertake such a switch lightly. However I would try a dog that was having seizures less frequently than every 2 weeks on Imepitoin as a first line therapy (ie before phenobarbital) I am so glad that you found the video helpful

    Hello doctor, thank you very much for this video. My dog, a 10 year 7 month old golden retriever, was treated with radiation therapy treatment (for brain meningioma) 4 months ago, and she has been on a mild dose of pregabalin 100 mg 2x 1 tablet daily (3 mg /kg twice daily) for the last 4 months. Recently, we observed a strange head twitching for half an hour; during this time, she was completely active and responsive. Could you please advise if it could be a side effect of pregabalin that showed as a myoclonus-type seizure? If Pregabalin is tapered down and eventually stopped, will it help her? The other anti-epileptic drug my dog takes is Levetiracetam 500 mg 3x 2 tablets daily. Please suggest. Thank you very much.

      I am afraid that our Royal College of Veterinary Surgeons ethical guidelines (quite rightly) prevent me from making comments on medical management of patients that are not under my care so I can only refer you to your veterinary neurologist to discuss this. This because i cannot safely comment without knowing more about the dogs medical history. Yes pregabalin can very rarely be associated with movement disorders but there are many many other explanation including a benign idiopathic head tremor so speak to your vet.     Thank you for an excellent presentation. I have a question. Are UC-II collagen supplements contradicted when a dog is on long term Pregabalin treatment? The literature does not seem to address that question. Thank you

      I have not had any problems giving specific canine and feline joint collagen nutraceuticals (available in the UK )with pregabalin. obviously the ones that I advise for my clients in in the UK are ones that have a higher level of science to back them up (rather than ones intended for humans etc)

    Thank you for the informative video! My doxie was recently diagnosed with lvl 2 IVDD. Took MRIs and decided to go the conservative route since her pain was manageable with medication and the hernia was relatively small. I’m already seeing a huge improvement in her walking after 5 days. Will be starting physiotherapy and hydrotherapy tomorrow 😊 Biggest difficulty this far has been keeping her resting, as she’s full of energy now that we’ve found effective pain management!

      She’s on robenacoxib and gabapentin. This combo seems to work really well and I haven’t observed any side effects beyond a bit of loopiness the first day on gabapentin.
