Physician Spotlight: Patricia M. Byers, MD, FACS
This spotlight is on Dr. Patricia Byers, MD, FACS. She is a Professor and Section Chief in Surgical Nutrition and Metabolic Surgery at the Ryder Trauma Center/Jackson Memorial Hospital in Miami, FL.
Dr. Byers became interested in nutrition while working in trauma and critical care, where she saw the impact of nutrition support on this patient population. She credits her career development to mentors like Drs. Stanley Dudrick, Bruce Bistrian, and Miguel Echenique.
Dr. Byers also discusses the importance of a multidisciplinary team in the care of patients receiving nutrition support, and how all members of the team are able to learn and grow from each other. She highlights the minimal nutrition training that surgical residents receive and yet surgical patients can benefit the most from nutrition support. In her institution, she makes sure that her surgical residents are exposed to their nutrition support team and gain experience in writing orders for TPN.
Dr. Byers is an active member of ASPEN and contributes to various committees, including the Physician Engagement Committee. She encourages others to get involved in ASPEN to promote their careers in nutrition.

    My baby cry’s so loud opening the air way I have a terribly hard time placing NG , it’s came through her throat and coughs and threw up and even coughed small blood , we will wait till tomorrow for a professional! I feel so terrible the blowing used to work for us but not anymore :(

    People whining over this: tube insertion is no or minimal pain and its uncomfortable, that's it. Feeding tubes can save the baby life and prevent malnutrition and have great results (if done correctly). Baby's cry over anything and everything, they also cry during diaper changes, car rides, outfit changes or when they are held still or when you take away their toy, does this mean these things hurt? Nope. It simply means they don't like it and they are uncomfortable, they will live! Also, to people saying this baby should be sedated, sedation comes with risks and side effects, especially for younger patients and they usually need to fast, sedation and making baby fast for 6 hours is NOT worth a procedure that causes 2 minutes of discomfort. 15:35 Notice how baby started to get fussy and cry BEFORE they placed the tube? He did because they were touching him and not letting him stick his foot up and they swaddled him, he turned into a beetroot and started cackling like he was being skinned alive, was he is pain? Nope. He also calmed down as soon as he was picked up. He is fine! 🙄

    I'd like to disagree with the entire procedure no matter what dignified intention you have. Does the procedure have to be so difficult, uncomfortable and painful that a baby like this has to cry unbearably? Are you telling me that there is no other way to get nutrition by mouth and have to rely on a tube that's supposed to allow food sustenance for period of time?

    While withdrawing catheter suction pressure should not be applied as mucus membrane may stick with the holes in catheter with negetive pressure.Mucus membrane may be damaged resulting in bleeding,resulting in further block of air way.Release pressure & slowly ,e.g,fem milimeter withdrawl than again apply pressure,suck secretion than before withdrawing catheter further, RELEASE NEGATIVE PRESSURE, & minimize chance of bleeding while suctioning.

    Poor thing sounded like he was choking I feel so bad they should’ve given him a sedative and isn’t it said that if the patient starts coughing to take the tube out and comfort the child and restart and maybe GIVE THE POOR THING A SEDATIVE!

      Sedatives are not given during this procedure because this is a painless procedure but it's uncomfortable if we give sedatives it may have some cause to the patient. Ok answer me that uncomfortable is just for a moment and by administration of sedatives may cause risk to patients life which is better??

    So many people commenting who obviously have no medical knowledge. This is not a painful procedure. Uncomfortable? yes, but not painful. Many babies cry just because they are being held still, not because of the procedure. These tubes are not placed for the fun of it, they are placed because it is a medical necessity. Babies need to be fed, and if they cannot take feedings by mouth, it has to be done in a different way.

    🤗Parents or Care Givers can also use a 5ml syringe of sugar water 2drop in 2the throat 2stimulate the gag reflex! it also helps the child swallow 2Help the NG Tube go down the throat more easily & the child can tolerate this Much BETTER! I've also seen children respond much Better using this technique & a pacifier when needed!🥰 The other suggestions don't always work 2stimulate the gag reflex enough 2Help the NG Tube go down it just depends on the child!🤗 My 24Day Old Son is in the NICU & will eventually be sent home W/a NG Tube! These videos & the classes I've been talking at the hospital have been Very Helpful! But doing the procedure is what is most Helpful & if parents don't feel comfortable trying on their child 1st then most hospitals will let the parents practice🤗 on a doll 1st! 🤲Im Praying🙏 4all of our Families 2have Good Health, Happiness & Wealth! 🥰

    Why is that brat crying at 15:40? She didn't place tube in yet, all she did was touch his face, stomach and didn't let him fold his feet into his face (I don't understand why he so badly wants his feet into his mouth, so gross). He literally lost it over nothing 🤣 Imagine a baby screeching and screaming over being touched. He seems like the type of baby who hates diaper changes but screams when his diaper is dirty (that way, you need to change his diaper. Damned if you do, damned if you don't) He is literally losing it over being touched. Watching a tube being placed in was so satisfying 😂 Look at his kicks at 17:06 🤣 He turned into a literal 🍅

    So thankful to this man. 12+ years on TPN and I'm so short gut I cannot survive without it. I've only ever had one line infection, I swim, shower almost daily, travel nationally and internationally (pre-COVID). Many think TPN is no life but I have achieved life long goals and have been blessed to see my neices and nephews grow up and start families of their own. Was lucky enough to hear him speak at an Oley conference and see pics of the famous beagles!

      That is a wonderful question. Other maneuvers to elicit the startle reflex such as someone in the room clapping hands can be used to stimulate swallowing.

I've also seen children respond much Better using this technique & a pacifier when needed!🥰 The other suggestions don't always work 2stimulate the gag reflex enough 2Help the NG Tube go down it just depends on the child!🤗 My 24Day Old Son is in the NICU & will eventually be sent home W/a NG Tube! These videos & the classes I've been talking at the hospital have been Very Helpful! But doing the procedure is what is most Helpful & if parents don't feel comfortable trying on their child 1st then most hospitals will let the parents practice🤗 on a doll 1st! 🤲Im Praying🙏 4all of our Families 2have Good Health, Happiness & Wealth! 🥰
